New Patients

To save yourself some time, we have our new patient packet available here for you to download and bring to your initial visit.  If you do not bring it your initial visit, we will have it available.

You may either email, fax or bring the new patient packet in person.

*Please understand that it is ultimately your responsibility as a patient to ensure that your carrier covers CHIROPRACTIC services.  We will gladly look up your benefits and provide you with an ESTIMATE, but be aware that your insurance company may deny payments and defer them to you as patient.  We will do our absolute best to give you the most up-to-date and accurate information possible, but please understand that in an ever-evolving healthcare system, we are constantly fighting to understand your benefits and what the insurance companies will pay for.

What form of payment do you accept?

We accept all major credit cards, Apple Pay, cash, and checks.

What should I wear to my visits?

Please wear comfortable clothes that you can move in.

How long should I expect to be in the office?

Our providers work hard to be as efficient as possible while maintaining a very high standard of care.  Depending on what your treatment plan entails, you can expect to be in our office anywhere from 15 minutes to one hour.  Ask your provider for a better estimate on your visit length.

What is your cancellation policy?

We require at least a 12 hour notice for appointment cancellations. Failure to give adequate notice will result in a late cancellation fee, no exceptions.
